Madison Soccer Club Parents:
The Morris County Youth Soccer Association (MCYSA) requires various forms to be completed in the correct manner in order for your child/children to receive player cards. Without a player card, they will not be eligible to participate in games.
Please follow the instructions below and use included links to complete required forms and provide them to your coaches.
Please read thoroughly and follow instructions carefully. The District Commissioner is extremely strict and will reject registrations that do not follow these requirements.
All signatures on all forms must be in ballpoint pen - no felt tip pen or pencil signatures will be accepted.
Player Membership Form
The Form must be completely filled out. Players AND Parents must date, print and then sign in script full first and last names—no nicknames allowed (i.e., Joe for Joseph or Kate for Kaitlin). The name that is on the birth certificate is the name that must be used. Do not complete the Team Name and Player Pass #, your coach will fill that in later.
Medical Release Form
The form must be completely filled out and notarized, with raised seal. Again, full first and last name, as they appear on the birth certificate, must be used.
SAGE (Set A Good Example) Pledge Form
Player AND parents must sign the same copy of the SAGE form. Again, full first and last name, as they appear on the birth certificate, must be used.
Proof of Age
Please provide a clear copy of the player’s birth certificate or passport with the Date of Birth highlighted. If certificate is in a different language, a certified translation must be provided.
Player Photo
Please provide a small current photo of the player, no bigger than 1” by 1”. It must be an original photo—computer paper-generated photos will NOT be accepted. DO NOT use photo from prior year ID cards.
